I was looking for a nutrition assessment form template that could be used over and over in LTC.  I wanted to be able to save assessments and then when they came up again, just change the weight and maybe a few other things.

I was wasting a lot of time, rewriting Dx, meds, diets, etc, ….things that never change.  I was doing my full assessments by hand and wanted to keep them in a file on the computer.

I have been a SureAssess user for 19 years.  It is comprehensive and user friendly.  My time in the facility is limited, and I have found that SureAssess is a definite time saver and produces a comprehensive, accurate, legible and concise nutritional assessment which surveyors love. With the time saved, I have more face time with the residents and ultimately improved quality of care. I have also found the format of the Nutrition Care Process easy to implement into nutritional assessments. The reason SureAssess is valuable in my practice is it gives me a “second look” at my residents.  I review the chart and visit the resident.  When I input my data, I have another opportunity to think about the resident and review my options for interventions.  SureAssess gives me a permanent record of my assessments should my assessment disappear from the chart.
